What to expect and complete

All new patients to the practice will have an initial consultation in the Headache Clinic with one of our headache physicians. Patients who meet PBS criteria for advanced therapies will be consulted and/or treated by Dr Rodrigues. All patients referred to our clinic are under the care of Dr Rodrigues.

To make an appointment, you must have a valid GP referral to Dr Julian Rodrigues and complete the New Patient Registration and Headache Questionnaire.

Please download the 4-week Headache Diary and diarise your symptoms for four (4) weeks prior to your appointment.
* Failure to do so may impact your medical diagnosis and treatment.

You may retrospectively complete the Headache Diary if you are already tracking your symptoms in another format e.g., calendar, journal or App. Please transfer only the information requested to our Headache Diary.

Directions and Parking

We are located on Level 3, Suite 303 Hollywood Consulting Centre, 91 Monash Avenue Nedlands WA.

There are 5 entrances to Hollywood Private Hospital and medical centres accessible via Monash Avenue. Please enter through Entrance 5 and follow the road all the way to the rear of the complex. You will pass the Hollywood Medical Centre on your right, the Hollywood Specialist Centre on your left before coming to our building, the Hollywood Consulting Centre. In the map here, refer to Building H.

Parking is available in the building on Level 1 and 2. Please avoid parking on Level 0 as this is 2-hour parking only. You will be issued a ticket at the boom gate entry which you can pay using cash or credit/debit card at any of the ticket payment facilities located near the lifts.

Fees

We are a private billing practice and fees are payable in full on the day of your appointment. We do not bulk bill nor do we assess/manage patients for MVA Insurance claims.

EFTPOS payment facilities are available, we do not accept cash or AMEX.

Medicare claims will be submitted by our staff and your rebate should be deposited into the bank account linked to Medicare within 1-2 business days.

Private health funds do not subsidise outpatient appointments. For an estimate of fees prior to your appointment, please email admin@suite303.com.au.

Appointment Cancellation Policy

Our practice requires 48 hours’ notice for appointment cancellations. A fee of $100 for missed new patient appointments and $50 for missed follow up appointments will be payable prior to rescheduling your appointment.
